Requirements
• Valid ID showing Manila address
• Barangay Clearance or Certificate of Residency
• Previous medical records of pet (if available)
• Pet must be brought to clinic (except for consultation on photos/videos)
How to Apply
Walk-in during office hours. No appointment needed but early arrival recommended as slots are limited (first-come, first-served basis).
